Who We Are

The Community Health Worker Network (CHWN) serves as a resource for the community linking individuals to community resources to improve the rates of chronic disease. The volunteer supports the Prince William Health District’s (PWHD) mission to have a community of healthy people, and works with the community to refer individuals to health care centers and/or provide lifestyle change programs on chronic disease prevention and management.

What We Do

We engage with community partners in order to utilize CHWs at community events or clinics. The CHW provides information in order to better manage, detect and prevent chronic diseases; such as diabetes, hypertension, heart disease, and stroke. Here are a list of what a CHW does: 

  1. Ensure that adult clients complete the CDC Pre-Diabetes Risk Assessment or go online to the website “Do I Have Prediabetes?” (www.DoIHavePrediabetes.org) in order to
  2. Identify and refer clients to community resources using the Aunt Bertha website - http://connectnorthernvirginia.org/.
  3. Promote and refer clients to lifestyle change programs such as the National Diabetes Prevention Program (NDPP) or Diabetes Self-Management Education (DSME).
  4. Promote and teach blood pressure self-management techniques, and distribute the Blood Pressure Self-Management Toolkit.
  5. Offer Road to Health Toolkit classes for individuals that are at-risk of developing type 2 diabetes.
  6. Promote preventive health services.
  7. Assist the Prince William Health District Chronic Disease Prevention Team during screenings and faith-based congregation events.
  8. Assist patients with health forms and answering questions that may include language translation.
  9. Help with patient follow-up calls.
